Private Insurance / Extended Health Benefits
Most private insurance companies provide coverage for the services of a Certified Athletic Therapist. As with other services, coverage differs between individual plans. Athletic Therapy may even be covered under your policy if you do not see it listed in your written description of coverage. We encourage you to check with your Human Resources Department and/or your policy provider to determine whether your policy includes the services of an Athletic Therapist.
If you discover that these services are not included, we urge you to download the forms located below and send them to the appropriate parties. Shouldn't you have the option to choose the therapy that works best for you?

Sport Accident Insurance for Athletes
If you're a club athlete, coach, official, or volunteer injured in your sport, these plans are a great bonus!
Minor hockey participants, including female teams, Junior and Senior level players, and officials of all levels, must use the CHA Injury Report Form.

All other athletes please use the AllSport Claim Form. (Please check with your Provincial Sport Organization to make sure that your sport has coverage.)
